This questionnaire will be sent to you when we receive your order and should take approximately 30 minutes to complete.
Felicity will then arrange a time with you for a 30 minute online video consultation to discuss your results and provide a personalised treatment plan. This will be geared towards the most favourable outcome through supplementation, dietary support and lifestyle advice.
